Appointments

Please arrive at each visit about 10 minutes early to update your personal records including current address, phone number, employer and emergency contacts. Insurance cards must be presented at every visit.

Emergency Care

For any life-threatening emergency, go directly to the nearest hospital emergency center or call 911. The OSF Saint Anthony Medical Center Level I Trauma Center and Emergency Department is always available, 24 hours a day, 7 days a week. It is located at 5666 East State Street in Rockford.

After-Hours Non-Emergency Care

For non-life threatening medical assistance after normal office hours, call your physician. A specially-trained nurse will take your call, evaluate your needs and recommend a course of action. Of course, OSF Medical Group patients may also utilize any of OSF's excellent PromptCare facilities.

Insurance Plans

OSF Medical Group accepts a wide variety of insurance plans, including Medicare and Illinois Department of Public Aid. Please call the office directly to verify that your individual insurance plan is accepted.
