Great News from Blue Cross/Blue Shield of Illinois! 

OSF Medical Group - College Avenue physicians are now participating in the Blue Cross/Blue Shield of Illinois (BCBSIL) PPO contract. 

Coverage extends retroactive to 9/13/09.  Please call your BC/BS plan administrator for more specific details.

Practice Overview

Location: 1701 East College Avenue, Bloomington, IL

Please know:

• Carle Clinic clinical and administrative staff have transitioned to OSF and remain at the College Avenue location.


• Medical records have been transferred to OSF Medical Group unless otherwise directed by the patient. Please call the administration office at (309) 664-3001 if you have questions about your medical records.


• Most patient insurance plans that were accepted by Carle Clinic, such as Health Alliance Medical Plans, are accepted by OSF.


• Primary and specialty care services will continue, including MRI, X-ray, mammography, laboratory, Convenient Care. Pharmacy and therapy, both services of Carle Foundation Hospital, will also continue.

Saturday:  By Appointment Only (acute care only)
